Adult Services

CSS provides supports and services to adults which align with the current service definitions in the DDA waiver. Multiple services may be chosen by an individual and used sequentially to provide the desired service schedule. The specific activities an individual prefers may be allowed in different waiver service definitions in the same day or across the week.

Adult Meaningful Day Services

Adults, 21 years of age and older, who are eligible under the DDA Community Pathways Waiver, receive individualized support services during weekday hours. Individuals choose a combination of services designed to promote independence and inclusion in the community including employment services and day habilitation services. Individuals choose to participate in volunteer and paid work experience, instructional, recreational, social, and community engagement activities. Individuals may receive continuous support in small groups or with 1:1 staff support, up to forty hours per week.

Personal Support Services

Children and adults eligible for the DDA community waivers, receive 1:1 staff support to pursue their goals for developing independence at home and in the community. Personal Support Services may include personal care, instruction in life skills, and social/behavioral needs. Adults may receive Personal Support Services in addition to Meaningful Day Services. Individuals are authorized by DDA for up to 82 hours per week of Personal Supports and may use services at any time seven days a week and during overnight hours.

Residential Services

CSS provides Community Living and Supported Living to individuals, 18 years and older, approved for Residential Services by DDA. For adults participating in community living, two individuals live in a townhouse or condominium located in upper Montgomery County. The residences are owned and operated by CSS. CSS provides the trained support staff and supervision as specified in each person centered plan. CSS currently operates 60 homes, and individuals have choices of the housemate they prefer and the residence best suited for them.

Individuals, participating in Supported Living, receive support to live in their own home, outside the family home. The individual may live alone or with a housemate, and is responsible for room and board expenses and the management of the home. CSS provides staff support as requested to pursue person-centered goals to increase independence and community membership.

Housing Supports

CSS provides housing supports to individuals participating in Community Living and Supported Living services. A certified housing specialist completes an assessment with each person and develops a support plan to promote independence.

Behavioral Support Services

Children and adults receive behavioral assessment, development of a behavior support plan, and ongoing monitoring of the implementation of the plan for those with an assessed need. Behavioral Support Services are provided in combination with other DDA funded services. Services are provided by licensed psychologists, BCBA professionals, and Registered Behavior Technicians.

Nursing Support Services

Nursing services are included in Meaningful Day, Community Living, and Supported Living services. Licensed nurses complete health assessments for each individual, provide training and supervision to direct support staff, develop nursing care plans, and delegate nursing tasks . Direct or skilled nursing is not provided through the DDA waiver.

Individual Characteristics for Success

CSS supports adults, 21 years and older, who are eligible for DDA funded services. Individuals supported have a wide range of skills and interests, including those assessed with behavior support needs, intensive staff support needs, and a need for specialized transportation.

Individuals with a need for continuous staff support, either in small groups or with 1:1 staff support, are supported to be successful in pursuing their goals.

Settings

Individuals receiving Meaningful Day Services often choose to participate in employment and community-based instructional services. CSS supports individuals to identify and obtain part time paid or volunteer work experiences in local businesses and nonprofit organizations. CSS supports each person to explore community options for recreation and social activities and to determine areas of interest to enhance community inclusion. CSS offers opportunities for small group activities, technology instruction and assistance, self-advocacy groups, hobby and leisure time activities, fitness and exercise focused groups, etc. in the CSS Resource Center. The CSS Resource Center also provides a reliable transportation hub for arrivals and departures to family homes or other locations.

Hours

CSS provides services on the schedule determined by the individual in the Person-Centered Plan as allowed by the specific DDA waiver service. Meaningful Day Services are provided every weekday either in person or as virtual service. A typical day is six to eight hours of Day Habilitation or Employment Services. An individual may receive personal Support services in addition to meaningful day services, further extending and individualizing the day. Personal Support Services may also be provided on weekends and holidays.

Transportation

CSS provides or arranges transportation to meaningful day services and to community locations where services are provided. Each individual’s team identifies the safest and most efficient mode of transportation to implement the schedule desired by the person. Public transit options are considered including Metrorail, bus, and Metro Access. CSS contracts with a transportation company which is authorized to provide rides through Metro Access. Additionally, CSS owns and maintains a fleet of minivans, and CSS direct support staff may drive individuals to scheduled activities. The CSS vans are tracked to provide additional supervision of community access. CSS has adequate numbers of accessible vans to accommodate wheelchair use.
